Whether you are coming in for the treatment of a specific condition, or to improve your overall health, the initial visit includes assessment and treatment based on your personal health history and current condition. Length of visit and fees vary according to the individual case. We are happy to discuss our fees with you. Your insurance carrier may cover office visits and some of our services, though many of those listed in this brochure are not considered "customary" and are not billable to insurance. Payment is expected at the time of service, however, if we are providers on your insurance plan, only co-pays and any non-covered item (i.e.: vitamins, herbs, homeopathics) will be collected. We carry appropriate billing receipts for reimbursement, if requested. We require a 24-hour notice of cancellation and reserve the right to charge for missed appointments. We are not Medicare providers, but do offer discounted rates to our senior patients.
If you would like to make an appointment, please call our office at (801) 373-8500 or contact us.
